Moving out of state

So right now I live at home with my grandparents and sisters. I'm 24 and have been thinking about moving out on my own but I freak out because where is live is just so hard to get started. I'm thinking about moving out of state and with my boyfriend. He's in agreement with me. I'm just wondering how will I know when I'm completely ready and what should I look out for since this will be my first time moving out. Btw I'm looking to move out within a year from now.
