There are many ways to put on a condom wrong. First make sure you tear the side of the condom packet (not across the top of the condom). Second pinch the tip of the condom (to make sure there's no air) and start to roll it down. If it doesn't roll down you put it on inside out, and need to throw it away, do not flip it over and reuse it. Third make sure the condom rolls all the way down the shaft of the penis. Four make sure to avoid using your nails which can cause tears. Five make sure your partner holds onto the condom when pulling out. Also make sure to check the expiration date on the condom (expired condoms are more likely to break), only use water based lube (oil based can cause latex to fail), keep the store the condoms in a dry cool place out of direct sunlight and never reuse a condom. I may have missed somethings but I can't think of anything else right now

Some mistakes are: condom is either too big or too small. When you put one on, you punch the tip of the condom so no air gets trapped inside, and so there is a place for semen to go. I'm sure people also don't put the condom all the way on, its supposed to go down all the way to the base of the penis.
