I am almost a nurse. That is not yeast. It's due to friction and moisture. You need to be sure and completely dry after showers. Put non fragranced lotion on it and try to wear loose fitting bras and shirts until it's healed.

I used to get that really often before my breast reduction. My breast created too much heat and was running constantly causing a rash that looked identical to that, I used to put a cold cloth on it and then dry it completely and rub coconut oil on it before bed, that way it can completely soak in over night, it ks antibacterial and it will make the dry skin go away. You can also try polysporn.

Are you wearing the right size bra? 
This looks like it hurts really bad and I'm unsure how it would get there. 
My first thought it an incorrect bra size and your breast rubbing together too much. 
I would definitely moisturize as much as possible
Maybe even go to the doctor
