Long distance and being ignored He keeps giving me an STD

Found out we still have the same STD and this fool is ignoring me! We fought before I left on New Years Day and he was upset because I wouldn't let him keep our 4 month old baby girl up North! He was supposed to be down south with is but he claims his other two kids need him too! Whatever! So we haven't had sex in 3 months and we finally slept together when I got up there! We just had Trich in October which he never explained how we got it again! Apparently he didn't get treated like a dumb ass! Now he's ignoring me and I am LIVID. I don't know what to do it's driving me nuts! He's avoiding calling to check on our daughter because he knows I will bring up the STD, what should I do? He literally just claims that he hasn't slept with anyone but me in the entire 5 years! That is bullshit but why is he lying after giving it to me twice
