Birth Plan, Hospital Bag, & Newborn Procedures

Okay, so I know labor and delivery can many different ways, but it's important to have what you prefer on paper. How many of you formulated a birth plan or plan to? At how many weeks did you pack your hospital bag? What did you put in it? And what are procedures that happen with newborns that I need to be aware of (example: hep B vaccine) ? I'm not asking opinions on the procedures, just wanting to know what all there is so I can include what I need in my plan for my baby.
