What are your must haves for a diaper bag?

I feel like I've over packed lol. What comes in handy for you more often than not (other than diapers and wipes of course)?

I have a huge bag lol and I always overpack because you never know!! My son is 3 months old and I always make sure I have two extra outfits in there at all times and a sleeper. Extra socks. Plenty of diapers plenty of wipes obviously. A back up pacifer. He always has one with him too. Hand and face wipes. Pacifier wipes. Toys. Usually two burp cloths. Diaper rash cream. Formula usually a small can. A bottle to mix formula in. Bottle liners(I use playtex nurser bottle that take liners.) A bottle, two nipples. A blanket. I might be forgetting a few things. Oh, and I always pack a thermos that I only use for warm water to make his bottles with while we are out and about. That way I always have semi warm good water to make them with. This is just everyday what I pack for going out and about around town. If we go shopping out of town or spending the day at someone else's house I might pack extra.

We have a backpack. In it it's EVERYTHING we need for an overnight stay if ever needed. From formula, bottles, pacifiers, to dispers, toys, baby wipes, 4 oniese-two short sleeve, and 2 long pant, long sleeves ones. A blanket, Tylenol and orajel for babies, her gas drops, her zantac. Her pediatricians business card. Etc etc . We have everything in case of any emergency was to arise. We take the backpack every time we take her somewhere.
Update- oh, and a on the go changing pad. 
Everything fits in the backpack perfectly. This is the one we have.

Two spare outfits, messy bag for dirty clothes, dummy wipes, surface wipes, a bottle of ready made formula, hand sanitiser, colic drops, bibs, muslin cloths, changing mat and the red book just in case. As well as nappies, wipes and sudocreme.
