NEVER had this happen, if a condom ever got lost, it's cause it was too large for the person wearing it. even if a girl did have a 'loose' vagina the condom should still be fit snug enough to not lose the condom.

When you have sex your vagina lengthens when you wait until you're not aroused anymore it returns to normal length & you can get it out

My sister had this happen to her twice actually, she first had a close friend try to 'fish' it out. Then she went to see saw a doctor for it and they told her to just wait until it just naturally comes out. And it did eventually. Best of luck to ya.
