transition from rock and play to crib

Any tips for transitioning baby from rock and play to crib? He's four months and takes naps in his crib but only sleeping on his side with a blanket right at his face. I'm scared for him to sleep like that while I'm sleeping but i would think he would move his head if he can't breathe at his age. He moves his head really week. I'm nervous
