Does it hurt to masterbate with a tampon in. If it does, then how do you guys masterbate with a pad on without getting blood all over you?

First of all, if you're going to be penetrating yourself, do not keep the tampon in. That can do damage to the inside of your vagina and your cervix. 
Secondly, if you don't want to get blood everywhere, just masturbate in the shower. Almost no cleanup. 
Now, you can masturbate using clitoral stimulation only with a tampon in, and depending on how heavy your period is, you could also do that with a pad on as well. 
But doing it in the shower is your best bet so it doesn't get too messy.
