Thrombose Hemorrhoid. HELP

I've had hemorrhoids before but this one is extremely painful. I've had it since I was 12 weeks pregnant, I'm now 33 weeks and it just started becoming an issue. I'm using preparationH cream and witch hazel wipes but I just noticed today it's slightly leaking blood mixed with pus. Idk if there's anything else I can do. Any advice would be appreciated. Sorry for the TMI
