condom slipping out

Okay so, after my boyfriend and I have sex, & he pulls out the condom slides off and stays inside of me. Does this happen to anyone else? And could I get pregnant because this keeps happening? I feel like it's because I tighten up (not purposely) therefore the condom comes off inside of me. (Not completely some still comes out)
