Excessive uterine bleeding

So, I have been bleeding for two months almost. I went to the doctor. They did an ultrasound and found nothing. They perscribed me an antibiotic and Provera for 10 days to stop the bleeding temporarily. After 10 days, the bleeding came back very strong, and now I'm in a lot of pain, as well. Any idea what might be the issue?
