Anyone ever filled out a continuing disability review for social security benefits? I got my 3 year review long form to fill out and was curious if I put all doctors, including giving birth, on there. Along with all medicines. Or if I only put the treatment for the condition causing me to be unable to work (9 total hip surgeries, 3 of those surgeries were full hip replacements). All within a less than 3 year span. I've lost so much bone and muscle from my left hip, that I will never be fully functional in that joint. Not to mention the pain. I did wean myself off narcotics though because I started feeling dependent. And i Found out a couple weeks ago my hardware is starting to loosen again (this is the second time it will have been revised for that reason, other 2 times were severe infection ).Help please!
