You'll only be able to tell by ultrasound. I have had two sets of twins and you would not have ever known. My sister in law gets considerably larger than me when pregnant, but has only ever had one baby at a time.

To be honest, looks like you're breathing out and trying to see something that isn't there. At 5 weeks you're not gonna have a bump that shape, twins or not

Baby is the size of a poppy seed and uterus is extremely low. You would not be showing, even if it were quadruplets.
