Then after 12 hours, you take it out, empty it and rinse it off.

I've been wanting to try one but I'm not sure about it. I'm used to tampons and I rarely have an issue or leakage with them 
how reliable is the cup? can you sleep/exercise without any problems?

It's a small cup you insert into your vagina to catch the blood and when it's time to take it out you just dump it into the toilet rinse it and put it back in.
