Overwhelmed mom or bad parenting?

So I was at Once Upon a child store today with my daughter... it was quite busy. There was a mom there with 3 kids, two boys probably under 6 and a baby girl. She was getting frustrated as the one little boy was spilling snacks and trying to climb into a baby jumper... so as my daughter and I are heading out the door I see the same boy standing in the middle of the parking lot peeing... his mom is still inside... what would you do? What would you be thinking about the mom at this point?

...just a few notes... the kids were well dressed and the boy was unharmed... he came back inside with me and we found his mom.
