You should try the wubanub paci.. works like a charm to keep it in and it doesn't get lost since there's a little stuffed animal attached to it.

I always hold my son's in at first. But if he wants it he holds it in himself and if he doesn't he spits it out or knocks it out of his mouth.

Yeah because they just forget about it and it falls out. My baby sucks on his maybe for a minute before spitting it out. Then might have it again or refuses to open his mouth, then I know he just doesn't want it.
