Cramps and heavy bleeding

I'm on my period and have really bad cramps to the point that I fell nauseated and can't walk, I've also been in my period for three weeks straight and I've been bleeding so hard that I can't leave the house because I fill up a tampon every 30 minutes.

Does anyone know what might be happening?
