Mil babysitting woes

I recently was offered a manager position at my job where I’ll have to double my hours from 20 to 40 a week. We really need the extra income and I don’t want to let my employer down. My 11 month old will have to go to my mil house every weekday that I work. My mil is super sweet and has 4 kids of her own (all are grown except her 9 year old). The problem is that she has a filthy house that STILL isn’t baby proofed. She has 3 dogs (2 labs and a yorkie) who are all badly trained and barely groomed. She also had to take in her husbands mother who has very bad dementia who needs constant supervision. It’s so bad that my mil just found a huge stash of used tissues, paper towels, and toilet paper in her bedroom so she’s very unkept and always wants to feed/hold my baby 😳. I don’t trust where her hands have been... my mil feeds him things I don’t want him eating like candy even though I pack snacks for him. The house is always in such chaos when I’m there with my baby and I honestly don’t trust him to be there anymore. Every time he comes home from there, he is filthy, covered in dirt, dog fur, and his clothes constantly get ruined (they never use a bib during feeding). I really have no other babysitter options. What do I do? I’m afraid to confront her because she will get so upset I just know it. The picture is of his socks after a day at her house.
