The teeth grinder in sleep

Okay, so my fiance grinds his teeth at night. I swear it is a million times worse then nails on a freaking chalk board! He's tried a mouth guard before and not only does it end up falling out of his mouth in his sleep, it's extremely uncomfortable to him.
