Has anyone who uses the “withdrawal” method as birth control ever gotten pregnant from the precum?

It's very unlikely to get pregnant from precum because it contains very small to none amount of sperm, if a woman gets pregnant using the withdrawal method is because partner didn't pulled out on time.
