37 week app

I was in L&D; on Monday for early labor but my contraction's went away. She checks me today at my app and says I'm dilated to almost a 4. I've been having contractions on and off today but I've been having MAJOR cramping and back pain that has been consistent all day. TMI - my vaj hurts sooo bad. There is so much pressure and it's burning. My app was 4.5 hours ago. Could I have dilated farther since then? Should I wait to call because I'm not having contractions? My cramps hurt really bad like I'm about to start my period. 😰
