How to ask my boss for time off for breast implants?

I am getting breast implants and labiaplasty and I’m not sure how to ask my boss for time off? 
Should I tell them exactly what I’m doing, or just say I have an elective surgery coming up? I work in a small medical office running insurance claims and coding so I think they’ll find out either way... I just dont know how to ask? 
Advice? (I’d need a week off)

You could just say you are having a procedure done. It’s really not your boss’s business what it is.

I don’t think you legally have to tell him what surgery you’re having, as long as you have a doctors note

I felt good enough to get back to Law school and semi normal life after a few days ... I only had the breast implants but it was super easy and quick recovery ... takes some time to wear a normal bra and lift heavy things - but I could drive and go to class carry my books etc after a few days

use your vacation time if you have it. it's not something they have to accommodate
