It can happen. The pills main purpose is to prevent ovulation so if you’ve already ovulated you can still become pregnant. The morning after pill does also change the ph of your uterus and put a lot of hormones into your body so it can also prevent a fertilized egg from latching on and growing. I have known a couple people that have gotten pregnant after plan b though. It’s most effective if you take it ASAP after the unprotected sex

There's 4 main reasons why morning after pills don't work, taking it after you've already ovulated, taking it past the 72 hr mark, taking it excessively (more than 2-3 times a year) and being over the weight limit

I’ve not, but it can happen. 
The longer between taking the pill and unprotected sex, the less effective it gets. Illness such as sickness can also make it ineffective :)
