I used a known donor and my situation was a little more smooth than than most because he was a personal friend. I typed a document stating that my baby was conceived under donor/recipient contract and that the donor has no responsibility to the child financially or otherwise. I also types a document stating that the donor was simply that and could never try to contact the child or disclose the fact that he was a donor and to the public should remain anonymous. I had them notarized and both us and the donor signed and got copies if both documents.

my wife and I are using a donor! we spoke to a lawyer and I got a pre typed document, we went in and both signed it and got it notarized.

Our clinic requires a legal contract. We paid $750 for the attorney.
