postpartum sex

so I'm 3 weeks pp and we have been extremely horny...so we said what the hell...we used a condom and had the best since i was 5mths preggers...so it felt so good until I felt leaking..i was thinking no i haven't bled in over a week...he pulls out the condom wasn't there he had to fish it out....now we're on panic mode.. we already have a 19mth old....we should've waited 😕...now I'm embarrassed to go back to the ob and have to tell her...im due to get nexplanon in 4 weeks....hopefully I have no worries...rant over....what would you ladies do?
