WTF is this?

Hello! First time posting on here! Below are a couple of pictures that I just took a few minutes ago. The first picture is a close up of what I found when I wiped after the using the washroom. I have had Mirena since September 2017 and, although its necessarily the safest, my boyfriend pulls out. I took a pregnancy test this morning and it was negative. I have no idea what the heck this thing is; I've never seen something like this before. Whatever this thing is, it has blood vessels starting to grow. I have not had a period for about 3-4 months, but I have been taking tests at least once every calendar month. Before I left work, I noticed that I was a little crampy and was spotting and thought nothing of it. Can anyone help identify what this is? I already have a doctor's appointment tomorrow morning so I'll talk to my nurse tomorrow, but I am concerned whether I should go to the ER or not. Help?
