You could have just had a coldsore coming. I suffer from cold sores, have since I was little and sometimes they pop out of nowhere. It doesn’t mean it’s because of your Boyfriend. 
Underwear doesn’t cause cuts or sores though. I suggest you both get tested to rule anything out and in the mean time don’t have unprotected sex or unprotected oral sex.
