Overmedicating Patients

So, I've been my mother in law's caregiver for the last 4 years. She's been of sound mind the entire time so it was mostly running her errands, taking her to appointments, doing the cooking and cleaning and taking care of her pets since she couldn't really do much physical activity. In March she went to hospital with an infection that lead to pneumonia and they had her for just over a week. She hasn't been right since then. Now I've taken over her medications and I'm not the type to just do something without questioning it, so I've googled all of her medications and interactions and whatnot. And I was amazed to find that she seems to have been taking all kinds of things that were not only unnecessary, but extremely dangerous. An example is her anxiety medication and her pain medications both cause her airways to constrict, and that's not good for anyone, let alone a person with severe COPD.

My question to you, is do you think doctors are over medicating their patients? Bonus question: Do you think doctors take advantage of patients who do not have someone to advocate for them when they are not of sound mind or just don't think to question their doctors?
