Mommy’s I need your help !

So my son he’s 13 months and he has been having a fever that goes on and off since yesterday afternoon. He’s head is burning hot it won’t go away. I have been giving Tylenol. If I take him to his doctor they don’t check him they just put a catheter in because he was born with kidney problems and that’s the drill they do every time he has a fever. But if they don’t get any pee out they end up sending me to the children’s hospital ER and doing the whole catheter again. What should I do ? Just go to the ER?
