Need help

I’ve chosen not to continue to vaccinate my son and I’m not really sure how to approach his pediatrician on the subject so I need a little advice ladies, do I have to sign a form saying I’m refusing? Can children services take my child for not getting him vaccinated? I’ve heard so many horror stories about this happening to other mothers who signed a form and I can’t imagine losing my baby, I’m a complete and total wreck over it.

(UPDATE) I didn’t have to sign anything my sons doctors is perfectly okay without it and without vaccinations❤️
