Bleeding after sex...

I’m gonna try to make this as short as possible. So I’ve been having this problem for months now & it all started whenever I was peeing a lot. Now I know its normal to bleed after sex if you have a UTI, but I never had one. I went to the Gyno 2 times & they didn’t find anything wrong with me, they did an internal exam & everything. The peeing went away & so did the bleeding for a bit, but now it’s back except I’m not peeing a lot. I had sex last night & went to clean myself up afterwards & there was a little bit of blood. I also noticed that he has to put it in really slow at first cause it feels like the outer part of my vagina hole hurts if he puts it in too fast. Sometimes he rushes into sex so maybe it’s just because he’s big & im not really wet yet? We use lube & it doesn’t seem to help. I’ve been dating him for 4 years & have never had this problem until a few months ago. Any idea what this could be? Does this ever happen to any of you?
