Failed first FET. Straight to second?

Has anyone had success with a second FET after a failed first FET?

We did our first transfer this month and it failed. We did <a href="https://glowing.com/glow-fertility-program">IVF</a> in July and froze all.

I am not sure what to do next! My doctor suggested either another transfer or another round of <a href="https://glowing.com/glow-fertility-program">IVF</a> with PGS testing. His choice is <a href="https://glowing.com/glow-fertility-program">IVF</a> because he thinks I may have egg quality issues. Insurance covers the <a href="https://glowing.com/glow-fertility-program">IVF</a> but with testing and freezing it will be about 7-8k.

He mentioned ERA but doesn’t suggest it until two failed transfers.

I just don’t understand why he would go right to another round of <a href="https://glowing.com/glow-fertility-program">IVF</a> without at least trying to transfer my other two frozen embryos. He doesn’t want to thaw and test them due to potential loss of quality.

A little backstory:

We started with my RE after 9 months of TTC and one chemical pregnancy. I had pelvic pain and went to my ob who discovered I had a separate uterus and referred me. After two surgeries to correct my septum we went right to <a href="https://glowing.com/glow-fertility-program">IVF</a> because insurance approved it but denied <a href="https://glowing.com/glow-fertility-program">IUI</a>.

With <a href="https://glowing.com/glow-fertility-program">IVF</a> we got 25 eggs, 17 mature, 11 fertilized, and 3 day five AA blasts.

I would suggest another FET since you have embryos frozen. We got pregnant with twins on our second FET. Good luck to you!!!

I’ve had 2 failed transfers, 1 frozen, 1 fresh. I actually got a second opinion about the era and said it wasn’t needed, they are usually needed if you have multiple losses with transfers to check for receptivity. I had to go through 2 retrieval’s, the first we only had 1 make it to blast the second we got 4 so now we have 3 frozen. I’m getting ready for a FET as soon as day 1 gets here. I would do another FET if I were you. Good luck!

Do another transfer! I had a failed FET the first time and two months later was pregnant with my second FET and no change in protocol from first to second. However, I had PGD done on my embryos before. You could always request PGD on your remaining embryos, but keep in mind that the thaw/freeze cycle isn’t perfect and about 5-10% don’t survive that. So it’s a bit higher risk to thaw, test, freeze, and then thaw again for the transfer. Good luck!!

I would personally do the ERA first before doing another FET. I just did my ERA in October and found out that I need an additional 24 hrs of progesterone before transfer for my uterus to be at optimal receptivity. Without knowing this valuable info, we would have transferred our precious embryos too early. I wish more RE's would advocate for this test because then you would at least have peace of mind that you are transferring at the right time. The ERA cost me $795 plus the meds, which to me was way cheaper than having failed embryo transfers for something as simple as timing. Our transfer is coming up November 27 so fingers crossed our embryos stick 🤞hope this helps, as this is just my perspective after having having a failed transfer and having to redo IVF again due to having no more embryos.

I personally would do another FET first. I wouldn’t want to put myself through another IVF cycle knowing I had frozen blasts that I could try transferring first. But that’s me! I’d ask a lot of questions and then do whatever you feel most comfortable with.

I don’t think you need an ERA yet. Sometimes everything is perfect but the transfer still doesn’t work! I would probably do the FET first without thawing/testing the embryos (it does risk damage). I’d only go straight for another retrieval if I was older and they were worried about diminishing ovarian reserve or something. I’m sorry your first FET failed - best of luck on your second transfer!

Maybe ask for the era and then do a transfer. I’m waiting to do an era now. I’m on my second I’ve round. My first and all 3 transfers were a fail. This time I tested my embryos. I had a coworker who had a failed transfer and then her doctor did an era and she is now 34 weeks. The era showed she needed a couple more days of meds.
