When can you tell if it’s diahoria with a new born?

I’m exclusively breastfeeding and my four week olds nappies are really runny almost watery yellow. Spoke to the doctor and he says this is normal? But when can you tell if it’s diahoria?

Breastfed babies have watery yellow poops usually. I wouldn’t be concerned unless they seem dehydrated or looking ill
