Is it normal for doctors offices/ clinics to not have a voicemail?

It's 4am and I just wanted to leave a message to say that I wouldn't be able to make it to my appointment in a couple of hours so I could get the sleep i missed and not worry about it but I guess not

Possible,  absolutely.  Most businesses like that use an after hours answering service.. but if the receptionist forgot to forward the phone lines when she left then the phone will just ring. It can't forward your call to the answering service if it wasn't set to forward, unfortunately.
