White specs in toddlers nappy

I've just changed my son (16m) for the first time this morning and he had some white specs on his penis and in his nappy. I cleaned him carefully and noticed it was worst on the tip of his penis and he seemed to be in some discomfort.

Any ideas what it might be? I've had a little look online and am still none the wiser. Some things suggest letting it clear up naturally and others suggest medical attention.
