Why can’t it be both? My children have names from my family that sound nice and flow with their first names.

Depends what the name is. Also depends what the first name is. And it could be both.

I did my best to give my daughter a middle name that was as close to a family name as possible-that complimented her first name. 
My name is Eva, I was named after my grandmother, Evalyn, my daughters middle name is Eve
