Starting femara?? When?

Question

I’m 38 have been try for 5 years now! I am starting femara but I guess I’m confused on when day 1 should be! I spotted yesterday woke up spotting a little after intercourse then turned into light bleeding is day 1 yesterday or today? I need to call for my 10 day ultrasound and start on day 3 on 5 mg! I feel hopeless and praying after so many miscarriages this will work! Any help or advice is appreciated!

Day 1 is full on bleeding not spotting, I’m on my 4th round of letrozole currently.
