It would be a General or honorable if that’s the only reason for the discharge. I’m an Human Resources Officer in the army

Depends on if you fail it in a row and if you aren’t improving. It ranges if there are other issues as well. If it’s a pattern, then a pattern of misconduct could be an issue but it’s hard to prove and ultimately up to jag.

My husband has failed PT tests a few times. All he had to do was go work out with some soldiers before formation. Hes still in and hasn't gotten an article for failing.

Well there's lots of different discharges from my understanding ( I'm an Air Force wife so I know it's different and I'm not active duty so don't take my word on any of this) but paper work isn't necessarily you getting kicked out at least for AF. Sometimes it's pretty much a paper telling you get your stuff together or your on the fast track out and typically it comes with help getting your pt scores up first before they just kick you out. Or if this is an on going thing and they've taken all the steps they can to get your pt scores up and you've received several warnings. 
Again I'm just a spouse so don't take my word as facts, and it's a totally different branch so there's that too. I'm only going off what has been explained to me

Didhonorable discharge is equivalent to a felony in civilian world they won’t do that to you you’d just get admin separation
