The condom probably had a small hole or broke, and if you’re on antibiotics or certain medications it can make birth control less effective.
It’s about a one in a million chance that you would get pregnant if you’re on birth control and used condoms...
Birth control is around 99% effective and condoms are 98% effective.

They could not be taking the birth control correctly, they could’ve taken an antibiotic which cancels out birth control. The condom could break.

I was on birth control and we used a condom. The condom got stuck inside me and it just so happens my birth control didn't want to work that week. It happens. Nothing is 100% effective.
