Ketone Problem

This is my first pregnancy and I knew before I got pregnant I was insulin resistant and I have thyroid disease. However I have been on metformin for years and thyroid medicine and I do see a dietician at my doctors office. I have been told to check my ketones of a morning and they are negative then throughout the day they go to small or moderate. The dietician has put me on a diet to have 15-20 carbs for breakfast and 45 for lunch and dinner with 15 carb snacks in between each meal and a 45 carb snack before I go to bed. I have done everything she has told me to do and my ketones are not changing, she also explained to me my ketones staying like they are can cause my baby’s brain not to develop like it should do now I am scared to death. Any advice or similar situations please?
