Maternity Allowance vs. SMP???

Question

So I started my job Jan 1st 2019, and based off the Gov site I'm not entitled to SMP but I can get the 39 weeks MA and 52 weeks MatLeave. 
My question is, my employer has already sent off my MatB1 form to HR for me at my job so they can send back the SMP1 form, but then do I have to send off for the MA1 form to HMRC?? Or is that another job for the employer?
Also I'm a little confused as to how much MA is per week and when it begins? Gov site has too much info half the time so I'd love to hear from some of the mum's out there who've done the process themselves.

I work 3 zero hour contract jobs (teaching supply agencies) and knew I wouldn't get SMP from any of them when I sent them my form.
I got posted an SMP1 form from each job stating the reason I didn't qualify for SMP.
I sent in the form, my payslips to prove I'd worked 26 weeks within the last 66 weeks (test period), alongside the SMP1 forms and my MAT B1 form and waited for about 3 weeks for their reply.
I got the standard Maternity allowance of £148 a week for 39 weeks which just started last week. I did have to call and chase it up a few days after though as they missed the start date I requested but she sorted it over the phone and it was back paid

I’m self employed and applied for the MA you can choose when this starts by when you intend to go off. 
The form has all the instructions and you just send it back, there’s a 6 week wait for a decision and you have to have paid a certain amount of national insurance for the year before if you want the full amount of £140 or so a week.

Not sure if this is the information you need! But I’m self employed so don’t get any other help but the government will give me which is just under £140 a week! I’ve sent of my paperwork and ask it to start on the 8th of July but havnt heard anything so really not sure how it all works

So i am applying for maternity allowance too. You print off the form fill it in and put in 13weeks of payslips and your matb along with the smp1 form. The smp1 needs to state when you plan on starting maternity leave. Make a copy of your matb because you have to send the original. Post it off asap because it can take 6 weeks. Ive had to sort this because I need to be eligible for it even though im only taking 2 weeks of it so hubby can get shared parental leave on full pay for 6 months. It’s a fuss.
