Should his name be on the BC?

Question

As a single mom should i put father name on birth certificate? 
We split up when i was 7months my baby is due any day now. 
We are on the whole just call when its time to go to the hospital situation .
He probably will be there in the hospital. 
He lives close by. 
No drugs or issues 
Im just concerned like if i put his name than i cant do anything with out his concent and need him to authorize everything. Is that true? 
WHAT SHOULD I DO?

I was told by a social worker In The hospital  not to put him on since our relationship as parents wasn’t great. She said if he wants to be on it, then he can take me to court and at least that way, he will have to pay child support.

In my state, a single mother can't actually put the father's name on the birth certificate - he must fill out a voluntary paternity declaration form first. If you two fill out that form and have his name added to the birth certificate, he will have a legal right to the child. Meaning, if he is watching the baby and for some reason refuses to return the baby to you, your only recourse is to get a signed order for placement from a judge. Unless you have a solid, trusting friendship with this man, go through the court system first.
