Down’s is determined right at conception since it is a chromosome abnormality. Nothing after conception will determine Down’s.

Having high hCG does not increase the chance of Down syndrome, that is typically decided at the moment of conception.  However, having persistently high hCG in the second trimester is one of the screens they use to determine if your baby is at a higher risk of already having Down syndrome (part of the quad screen).
