Hopefully just the picture but dont you have a proper 20 week scan scheduled where they look at all this stuff? I had mine friday (not 3d) and they can tell if baby has deformities or cleft lip/pallet by the scans and tell you. I'd ask for a proper scan cause this photo kinda worries me also. But 3D photos are also bad for this type of thing and making you see things that are probably not there

Looks like it’s just the picture, he could have been moving when they took it. I got one where my little ones face looks lumpy around his mouth but a few moments later looked fine.
