If anything is too personal, you don’t need to answer. I completely understand if you don’t want to share everything. We are just trying to see every side of things so we can determine what will work best for us. We sincerely appreciate all feedback.

1. What were your factors in choosing a donor sperm - Did you use known donor? If so, why? If not, why not?   
2. If you went through a known donor, what steps were taken to ensure all legal rights were terminated at the time of donation. 
3. If you went through a cryobank, what were the positives/negatives? What could have gone better? 
4. Why did you choose the bank you went with? Would you recommend them?
5. If you did IUI’s, did you do them at home or at the doctors office? Why?
6. IVF users.... Was it reciprocal IVF? why,why not?
7.  Would you do anything differently regarding the processes taken, now that you’ve done it? 
8. What steps were taken/needed to ensure second parent was legally the second parent? IE, in the worst case scenario during childbirth/thereafter, what was needed to make sure that you were the parent? Was second parent adoption needed?
9. Where did you find the information to determine the steps to take? State to state differs, so just wondering where/who we need to contact.
10. What kept you positive during the negative test results?
11. What did carrier do to keep non carrier feel involved in the whole process?
12. What is something you WISHED you would have known when you started the process that you know now?
