Missed twins on initial sono?

I was wondering how often twins are missed in early sonos. Ladies having twins, we're you initially told you were having a singleton to find out later it was actually twins?

I saw a girl on here who thought she was having 1. Her 9 week scan only showed 1 and then at the 12 week scan there were 2.

Twins run in my family so I asked my tech this.  She's been at my practice for 15 years and said by 7-8 weeks it's certain whether or not it's twins and has never missed it.

My twins were picked up at 6w3d.
With this pregnancy (singleton) my consultant at my 7w2d scan refused to confirm there was just one baby - she said she would rather wait until the following week to confirm.
