Twins or one baby and one sac?!

Had our 8 week ultrasound and doctor is unsure if we are having twins or if she saw one baby and a sac. Anyone have something similar? I’m really hoping it’s only one 😭

I see one fetus in the sac but not sure what that other fan shaped object is. It doesn’t look a fetus tho.
