Is this a miscarriage? TMI GRAPHIC WARNING

I have had 3 failed doses of clomid and provera my last period was in May I haven’t been ttc since February so I haven’t been on any medicine since around April and have continued to have unprotected sex with my fiancé but haven’t in the past 2 months.

My periods have always been severely irregular and I have had small blood clots before but never like this and never has it been tissue severely larger than a quarter. My birth control started 9/15/19 and my period started 10/11/19 I passed these two lumps of tissue 10/12/19 after my cramps being so severe I refused to move out of bed which is unusual they are never that severe my bleeding has also been heavier than normal. After I passed the tissue I also fainted and I have been slightly nauseous. I didn’t take a pregnancy test due to the fact of giving up on ttc I figure if clomid don’t work then there’s no way I could get lucky enough to conceive naturally and have had no pregnancy symptoms, but my doctor is concerned it’s a miscarriage.
