Do you all go by this telling you that you are 3w&6d? if not how do you determine how far along you are? 
and when is good to go ahead and get the first ultrasound?

Why wouldnt you go off it? It's based on your last period. And call your doctor and tell them your last period and that you got a positive and they'll get it scheduled based on their policy.

It’s typically by your last period.  I think the doctors schedule first ultrasounds around 8 weeks because that’s far enough to finally detect a heartbeat. 
When you call for an appt they’ll ask the date of your last period to estimate a gestation. Then the ultrasound will tell you based on the size. It is common to vary 1-2 weeks.
